Documents for Proving a Qualifying Life Event

Health Insurance
Everybody can enroll in an individual health insurance plan during open enrollment. But sometimes events such as a birth or marriage mean you’ll require to change your coverage at another time of the year. These are called qualifying life events. After a qualifying life event, you have a period of 60 days to change your health insurance plan or enroll in a new plan. You also may be able to choose a plan up to 60 days in advance of some qualifying life events. This can be during open enrollment or once open enrollment has ended.

You can employ any of the following qualifying life events to enroll in one of our plans during a special enrollment period. For any of these events, please provide us with one of the documents listed below. Some events may need more than one document.

Acceptable Documentations

Loss of employer-sponsored coverage

  • Letter or document from the employer indicating the employer changed, dropped or will drop coverage or benefits for the employee, spouse or dependent, and the date coverage ended or will end
  • Letter from the health insurance company indicating coverage termination date
  • COBRA documentation indicating the length of coverage with beginning and end dates
  • Divorce or annulment papers as well as the ending of health care responsibility andproof of prior qualifying health coverage within the last 60 days

Loss of Medicaid

  • Documentation from Michigan Department of Health and Human Services showing the reason for the loss andwhen coverage ended or will end
  • Letter from Medicaid or Children’s Health Insurance Plan (CHIP) indicating when coverage ended or will end

Moved Out of Plan Area or Relocated into New Plan Area

Evidence of prior qualifying health coverage within the last 60 days and one of the following:
Documents for Health Insurance in Qualifying Events

  • Lease or rental contract
  • Mortgage deed if showing owners utilize the property as a primary residence
  • Front and back of the driver’s license or country identification card
  • America Postal Service change of address confirmation
  • Official school documentation, together with enrollment or housing documentation
  • Letter from current or future employer that you moved for work
  • Green card, education certificate or visa if you moved to the United States from another republic
  • Birth certificate or application for a birth credential
  • Adoption records or placement for adoption
  • Lawful guardianship document
  • Court order or child support directive

Marriage

  • Marriage license (consists of proof of coverage for at least one, subscriber or spouse)
