Just recently, Fortify Children’s Health and Anthem HealthKeepers Plus announced a new partnership that will put patients at the center of the healthcare system. Fortify Children’s Health, LLC and Anthem HealthKeepers Plus, Anthem Blue Cross and Blue Shield in Virginia’s Medicaid plan, have announced the formation of a new partnership that will bring value-based health care to more than 25,000 of Anthem’s pediatric Medicaid members within the Commonwealth of Virginia.
Models for Better Health Care
The value-based agreement demonstrates a national health care shift toward a more coordinated model focusing on quality and better patient care experiences. The model aligns Fortify’s vast network of primary care physicians, specialists and facilities caring for children with Anthem’s population health capabilities and primary care support programs.

In this program, Fortify clinicians have access to various tools, enabling real-time collaboration and care coordination. Fortify clinicians will readily identify gaps in Anthem members’ care and find tailored support that can deliver improvements across cost, quality, and outcomes by sharing timely and accurate data.
